Q: What is Post Acute Care (PAC)?
A:Post Acute Care is any skilled nursing and rehabilitation that occurs after a hospital stay. Typically, it serves to transition the patient to home assisting them in recuperation. It aids in improving care planning, and importantly, it increases positive communication between hospitals and the community based providers.

Q: What forms of Payment do you accept?
A:We accept Medicare, Medicaid, Private Pay, and Insurance. The Business Office will have a list of Insurances that the facility is currently in-network.

Q: Will Medicare Part A cover patient stay in the facility?
A:Yes, however, patients must be admitted after a three consecutive night admission stay in the hospital. Generally, during the first 20 days of care, Medicare will pay 100%. For days 21-100, a co-insurance rate is required. Medicare does not cover skilled nursing fees over 100 days.

Q: Will patients have private rooms or shared rooms?
A:Requests may be made for private rooms, they are not guaranteed, Based on our room capacity situation, we will make every attempt to meet your needs.

Q: Are their dedicated nurses that consistently see the same patients?
A:Yes, we try to consistently schedule the same nurses on the same halls so they can build a relationship based on trust and caring with the patients and residents on a consistent basis.

Q: Do you have 24/7 RN coverage?
A:Yes, we schedule RN staff coverage.

Q: How often will a physician or health professional visit?
A:We have a Medical Director, Attending Physician, and Nurse Practitioners that visit several times a week to ensure high quality and satisfaction for out patients. Nurses will be in constant contact with patients, and if requested, a patient can see a physician as often as needed.

Q: Will patients receive regular rehabilitation by Physical Therapists?
A:Yes, provided that physical therapy is within the realm of the care plan.

Q: When are the visiting hours?
A:We encourage family member to visit their loved ones between the hours of 7am and 10pm as your loved one will benefit from rest while recovering.

Q: Is smoking permitted in the facility?
A:This is a non-smoking facility. However, there are dedicated outdoor areas that residents are permitted to smoke at posted times.

Q: Can family members and loved ones bring the patients their favorite food?
A:Provided that the food is within the limitations set by our Registered Dietician and as stated in the plan of care, the patient's favorite food can be brought to them.

Q: Will patients have telephone and internet access?
A:Private phone access is available to every patient as well as internet access. The patient can bring his or her own laptop; however, if needed we will provide one for our patients on a case by case basis and based on availability.

Q: Will patient rooms have a television?
A:All rooms have cable TV accessibility. The facility does not provide a TVs. There are televisions located in the common areas of the facility.

Q: What kind of activities is planned for the patients during their stay?
A:We have active patients that are committed to making their time enjoyable. Scheduled activities include: music, fitness activities, religious meetings, art and bead therapy. If you would like to arrange something special or have suggestions and ideas for individual or group activities, please inform our Community Life Director.
